Mistakes that get New York apostilles rejected
- A NYC birth certificate without the Letter of Exemplification. It can't be apostilled. Order the long form with the letter.
- Skipping the county clerk. Most notarized documents and local vital records need county clerk certification first.
- The wrong county clerk. It must be the clerk where the notary is qualified, or where the record was issued.
- Short-form records. Many countries want the long form showing parents' names.
- FBI checks sent to New York. Federal documents go to the U.S. Department of State. See FBI Apostille.
New York City records, step by step
1. Long-form certificate
Order the long-form birth or death certificate from the NYC Health Department.
2. Letter of Exemplification
Request it with the certificate. It's what makes the record eligible for an apostille.
3. County clerk
The New York County Clerk certifies the signature on the record.
4. Apostille
The New York Department of State issues the apostille.
