Why CORI forms need a notary

The Department of Criminal Justice Information Services (DCJIS) needs to know the person requesting a CORI is who they say they are. A notarized signature confirms that. Organizations such as nonprofits, schools, healthcare employers and landlords may also ask you to complete a notarized form.

How it works

  1. 1

    Request an appointment

Tell us which form you have and where you'd like to meet.

  2. 2

    Bring your ID

A current government-issued photo ID. Don't sign the form before we arrive.

  3. 3

    We notarize

We verify your identity, watch you sign and complete the notarial certificate. You submit the form as instructed.

Using your CORI abroad?

A Massachusetts CORI is a state document that gets a Massachusetts apostille, not a federal one. We can handle that for you, including tracked shipping.

Before your appointment

  • Have the right form. Use the form your employer, landlord, licensing board or organization gave you, or the DCJIS form for your personal request.
  • Fill it in, but don’t sign. Complete everything except your signature. You’ll sign in front of us.
  • Bring a current photo ID. A driver’s license, state ID or passport that isn’t expired. The name should match the form.
  • Check for extra signers. If a parent, guardian or organization representative also signs, they need to be there with their own ID.

Who asks for a notarized CORI form

Employers

Schools, healthcare employers, daycare centers and others who screen staff and volunteers.

Housing

Landlords and housing authorities reviewing rental applications.

Licensing

Professional and occupational licensing boards in Massachusetts.

Personal requests

Individuals requesting their own record, including for immigration or travel abroad.

Need your CORI for another country?

If a foreign employer, school or immigration office asks for your Massachusetts criminal record, the CORI usually needs a Massachusetts apostille, and sometimes embassy legalization and a certified translation too. Once you have your CORI, we can take it from there. See CORI Apostille. If the country asks for a nationwide record instead, that’s an FBI background check, which needs a federal apostille.

Frequently asked questions

Do you request the CORI for me?

No. You or your organization submit the request. We notarize your signature. Once you have your CORI, we can get it apostilled. See CORI Apostille.

Do you serve all of Massachusetts?

We serve Greater Boston for mobile appointments. Ask us if you're farther away.

Is a CORI the same as an FBI check?

No. A CORI covers Massachusetts records. An FBI check is a federal record and gets a federal apostille. See Federal & FBI Apostille.
