MEDICAL DOCUMENTS
Medical Records Affidavits and Medical Letters, Notarized
Records custodians certifying medical records, patients signing authorizations and doctors signing letters for visas or use abroad all need a notary they can trust with sensitive information. We come to hospitals, practices, law firms and homes across Greater Boston, and we add the apostille when a medical document is going abroad.

Mistakes we help you avoid
- The wrong certificate. A records custodian affidavit usually needs a jurat, not an acknowledgment. We use what the document calls for.
- Blank spaces. We cannot notarize a document with blanks, so please complete it first.
- A signer without authority. The person signing should be the custodian or someone authorized to certify the records.
- Unsigned doctor's letters for use abroad. A doctor's letter going abroad needs the doctor's signature notarized before it can be apostilled.
